This has been in the family since 1914. Should I

leave the 107 year old patina, clean it up or refinish it?

Your ancestors sweat is squeezed into the stock in the grip area and had there faces of that stock for 114 years. Respect your ancestors.

Looks like oil stains, scratches and a decent gouge mark. Likely too much sanding involved to get down to level and clean wood. I’d give it a cleaning with mineral spirits, alcohol or acetone. Light sanding with 220 or finer along with a few coats of any wood finishing oil.
