model 12 release trigger new when set cant push button to unset
It is out of time. The action slide lock hook is hitting the lug on the hammer. It does not re-cock the hammer, the hammer is re-cocked when the action slide is released and slides the breech bolt over the hammer and resets the sear. Usually caused as the end of the trigger is worn away. It is called a short trigger. This is my guess without having it in hand.
