I bought this forearm a little while back, but never used it. I got it from the orignal buyer and he also never used it. It is a standard width and size for a type 4 forearm iron. Serial number is 95,xxx.
Now for the full disclosure. I wanted to try and see how it would look on my gun so I pressed it up against the barrels and a little more muscle than brains resulted in a very fine split at the very front. You can see it highlighted by the red arrows in the pictures. Otherwise the forearm is absolutely perfect. The line is approx 1" and does not reach the checkering.
I'm sure that someone who knows how to work with wood can fix it like new, but I just won't have a need for it.
