Your ejector trip rod is not retracting for some reason, or something in the forearm. Read on...
Remove the stock and look to see if the bent (90 degree) end of the ejector trip rod is sitting IN FRONT of the LH hammer. Also, when fired, the ejector trip rod is pushed forward by the LH hammer, and that "push" is what forces the rod end out the front of the receiver. The small part sticking out the front, then "fires" the ejector sear in the forearm. Once the gun is opened, the ejector trip rod is normally pushed back to the rear in the receiver.
You could have: (1) a stuck ejector trip rod, and/or (2) a worn ejector trip/sear #317 in the forearm, or (3) a failed forearm ejector sear trip coil spring #309. And, if someone has worked on the forearm, they may have not knowingly launched the ejector sear plunger #313.
